Incident(Flash Report)

Flash Report Investigation.

  1. Login as Nominated Investigator user and go to My Action screen.
    There are four section provided to view and take action against tasks which are mentioned below:
    • FOR YOUR ACTION: Display “Incident”, “HSE Observation”, and Corrective and Preventive Action items (CAPA) which needs to be performed. It also displays the tasks which are saved as draft. User will update final information and submit it for further action. (This section is displayed by default)
    • FOR YOUR REVIEW: Display the tasks such as “Incident”, “HSE Observation”, and Corrective and Preventive Action items (CAPA) which needs to be reviewed. The tasks will be displayed to reviewer for review where reviewer will take action against it.
    • FOR YOUR INFORMATION: Display all the tasks such as “Incident”, “HSE Observation”and Corrective and Preventive Action items (CAPA) to creator/requester with their current status. User can view the task details by clicking subject in “Subject” column.
    • UPCOMING TASK(S): Display future tasks which are planned already.
  2. When subject of “Incident” in Section column in “REVIEW ACTIONS” section is clicked “Report Incident” screen will be opened in different tab.

  1. There are 3 section in the screen [Image 2] title as mentioned below:
    • Flash Report Details: Where user can view the details of the flash report.
    • Investigation: Where Investigation Team Members shall fill up investigation details and take action against the selected classified flash report.
    • Safety Alerts: Investigation team can send safety alert to other regarding such Flash reports and try to prevent Incident at other location.
  2. Based on the classification of the flash report the Investigation Tab shall list the sections as below
    • Classification of Flash Report as Near-Miss
      • Section A : Flash Report Detail
      • Section B : Investigation
      • Section C : Near-Miss Cause
      • Section D : Statement
      • Section E : Improve & Control (CAPA)
    • Classification of Flash Report as Loss of Containment(LOC)
      • Section A : Flash Report Detail
      • Section B : Investigation
      • Section C : Near-Miss Cause
      • Section D : Statement
      • Section E : Improve & Control (CAPA)
    • Classification of Flash Report as Incident (Level 1, Level 2, Level 3)
      • Section A : Flash Report Detail
      • Section B : Investigation
      • Section C : Injured Person Details
      • Section D : Statement
      • Section E : Incident Cause
      • Cost Calculation of Incident/Near-Miss
      • Section F : Improve & Control (CAPA)
  3. Others sections are enabled based on the selection of “Nature of Injury” and “Incident Classification” option(s).

Review Process for Different Type of Reported Flash:

  1. For LOC, Near Miss And Level 1 Incident:
    To take action against a particular Flash Report related to “HSE” select action from drop down list in Investigation section:
    • Investigation Completed & Send for Review 1 : The Flash(Incident) Investigation report will be sent for review to the FHSE Manager/ Location Person Incharge

    To take action against a particular Flash Report related to “Security” select action from drop down list in Investigation section:
    • Investigation Completed & Send for Review 1: The Flash(Incident) Investigation report will be sent for review to the Location Security Manager or Location Person Incharge.
  2. For Level 2 (HIPO) :
    To take action against a particular Flash Report related to “Level 2” select action from drop down list in Investigation section:
    • Investigation Completed & Send For Review 1:The Flash(Incident) Investigation report will be sent for review to the Nominated Designated Team Member(Each team member Review required.

    Note: Each team member Review required.

  3. For Level 3 (HIPO) :
    To take action against a particular Flash Report related to “Level 3” select action from drop down list in Investigation section:
    • Investigation Completed & Send For Review 1:The Flash(Incident) Investigation report will be sent for review to the HSSEM, Country Manager and COO user role.

    Note: Each User Role review required.

Note:

  • Investigation Team can edit the report contents.
  • The fields marked with * are mandatory to be filled.
