Incident(Flash Report)

Identify HIPO and Investigation Team by Country Manager.

  1. Login as Reviewer (Country Manager) role user to identify the Investigation Team Member and go to My Action screen.
    There are four section provided to view and take action against tasks which are mentioned below:
    • FOR YOUR ACTION: Display “Incident”, “HSE Observation”, and Corrective and Preventive Action items (CAPA) which needs to be performed. It also displays the tasks which are saved as draft. User will update final information and submit it for further action. (This section is displayed by default)
    • FOR YOUR REVIEW: Display the tasks such as “Incident”, “HSE Observation”, and Corrective and Preventive Action items (CAPA) which needs to be reviewed. The tasks will be displayed to reviewer for review where reviewer will take action against it.
    • FOR YOUR INFORMATION: Display all the tasks such as “Incident”, “HSE Observation”and Corrective and Preventive Action items (CAPA) to creator/requester with their current status. User can view the task details by clicking subject in “Subject” column.
    • UPCOMING TASK(S): Display future tasks which are planned already.
  2. When subject of “Incident” in Section column in “REVIEW ACTIONS” section is clicked “Report Incident” screen will be opened in different tab.

  1. To take action against a particular Flash Report, select action from drop down list in Authorization section:
    • Reviewed & Send For Further Investigation: The report will be sent for investigation to the members selected to conduct investigation. Select Investigation Team Member and Identify Designated team Member(For Level 2 HIPO incident) & send the Incident report for investigation.
    • Send back for correction: To send back an Flash Report for correction. The report will go back to the Location Person In-charge user role who reviewed it.
  2. Click Submit button to send the flash report for further action as per the workflow based on the above selection of the option.
