Incident(Flash Report)

Approve Investigation by Country Manager/ HHSE&S/ DHSEM

  1. Login as Approver (Country Manager/ HHSE&S/ DHSEM) role user to Approve the Investigation and go to My Action screen.
    There are four section provided to view and take action against tasks which are mentioned below:
    • FOR YOUR ACTION: Display “Incident”, “HSE Observation”, and Corrective and Preventive Action items (CAPA) which needs to be performed. It also displays the tasks which are saved as draft. User will update final information and submit it for further action. (This section is displayed by default)
    • FOR YOUR REVIEW: Display the tasks such as “Incident”, “HSE Observation”, and Corrective and Preventive Action items (CAPA) which needs to be reviewed. The tasks will be displayed to reviewer for review where reviewer will take action against it.
    • FOR YOUR INFORMATION: Display all the tasks such as “Incident”, “HSE Observation” and Corrective and Preventive Action items (CAPA) to creator/requester with their current status. User can view the task details by clicking subject in “Subject” column.
    • UPCOMING TASK(S): Display future tasks which are planned already.
  2. When subject of “Incident” in Section column in “REVIEW ACTIONS” section is clicked “Report Incident” screen will be opened in different tab.

  4. To take action against a particular Flash Report Investigation select action from drop down list in Authorization section (Must be reviewed by all Reviewer as per workflow):
    • Investigation approved & CAPA assigned: The investigation report will be verified, if it valid on the approval by all the Reviewer roles. All the CAPA assigned in the Section Improve and Control(Suggestion) shall be assigned to the responsible person.
    • Send back for Investigation correction: The investigation report will found invalid or inappropriate it will be send to the Investigation team members to revise the report.
    • Change Classification: During the review of Level 3 (HIPO) Investigation, if required to revise the Flash Report Classification.select an action and Report move to Country Manager for Change Classification.
  5. Once all the assigned Incident Investigation Suggestions/Recommendations Actions is/are closed, Incident report shall be implicitly updated as “Incident Closed”.
